Short Term Care

We provide 24 / 7, 365 days a year short-term Transitional Care service that is compassionate and an innovative approach to caring for our community’s ill seniors. Focused on freeing up acute care and emergency department beds in hospitals, it promotes the use of community care services while also helping to ease the many stressors facing caregivers and families of our aging senior population. Our staff provides individualized attention and intensive support with all activities of daily living including bathing, dressing, medication reminders and nutritious meals and snacks catered to each client’s dietary needs and restrictions.
